Social Prescribing
What we do
Our Social Prescribing Link Workers help with the things that affect health but are not medical, such as loneliness, money worries, caring, housing, employment and getting active. They work with you on what matters to you, build a simple personalised plan, then connect you with the right local groups and services.
Who it is for
Any adult who would benefit from community-based practical or emotional support, including people with long-term conditions or mild to moderate mental health needs, or anyone who is feeling isolated.
How to access
Contact your GP practice reception and ask for a Social Prescribing appointment.
If you are unsure what to say, you can simply ask to speak to the Social Prescribing team.
What to expect
- A relaxed conversation to understand your goals.
- A short plan with clear next steps.
- Connection to local activities or services, for example community groups, benefits advice, debt support, exercise, volunteering or carers' support.
- Follow-up to check progress and problem-solve together.
