Learning Disabilities Social Prescribing
What we do
Specialist Social Prescribers support adults with a learning disability and their carers to access community activities, benefits and advocacy, and to remove barriers such as transport, form filling and reasonable adjustments.
Who it is for
People with a diagnosed learning disability, including those with additional physical or mental health needs, and family or paid carers. Local needs assessments highlight the importance of tailored support as people live longer.
How to access
Ask your practice for referral to the Learning Disabilities Social Prescribing worker in the PCN. If you need help getting started, the Personalised Care team can advise.
