Coaches help you build healthy routines that stick. Expect support with sleep, food choices, movement, pain management and stress. They use proven behaviour change techniques and set realistic goals with you.
Who it is for
Anyone motivated to improve a long term condition such as type 2 diabetes, hypertension, obesity, persistent pain or low mood related to lifestyle. Locally, coaches already work across Billingham and Norton practices.
How to access
Ask your GP practice for a Health and Wellbeing Coach appointment.
What to expect
- One to one sessions focused on what matters to you.
- A simple action plan and regular check ins.
- Coaching that complements, not replaces, your medical care.
