What we do
Care Coordinators are your single point of contact for complex journeys. They help organise tests and referrals, chase results, line up community support and make sure your plan is clear and shared.
Who it is for
People with long-term conditions, frailty, frequent admissions, or care home residents who need joined-up support from several services.
How to access
Ask your practice for Care Coordinator support. Many coordinators are based in Hartlepool and Stockton Health's Personalised Care service which works with our PCN.
What to expect
- A named contact who knows your story
- A personalised care plan
- Coordination of appointments, community services and follow-up
Why this team is great
They save you repeated phone calls, reduce duplication and help you get the right care at the right time. Local coordinators work closely with GPs, nurses and social prescribers which keeps care seamless.
